Please remember that according to official government rules in order to get
reimbursed for your travel expenses:
| You must travel to the US on a W/B
(waiver/business) or B/1 visa. It is important that the inspection officer at the port of entry either marks your I-94 (Arrival/Departure Record) with the waiver visa by circling W/B as an option or by marking your I-94 card with B-1 visa status.
We will need a copy of your
passport/visa to receive reimbursement. We will | be able to copy that information at the Montauk Yacht Club upon your arrival. Federal statutes and government
regulations require that certified U.S. flag | air carriers are used for all government-financed transportation.
Because Brookhaven National Lab (BNL)
has discount-rate agreements with | several car rental companies, car rental arrangements should be made, whenever possible, by the BNL Travel Office. If you choose to rent on your own, please request a tax-exempt form from the Conference Coordinator and do not take out any additional insurance.
Please fill out and submit BOTH registration forms |

| BNL guest registration |
| Conference registration form |
We will cover your economy travel expenses (flight and transportation to and
from the hotel) and up to four nights accommodation (including all meals) at
the
Montauk Yacht Club Resort and Marina.
We look forward to seeing you in Montauk.
Sincerely,
Eileen Levine
